The Service provides health education, wellness consulting, functional health analysis, lifestyle guidance, and educational programs.
Unless and until a formal doctor–patient relationship is explicitly established in writing under applicable state law, all offerings are educational, informational, and consultative in nature.

4. Licensure & Scope of Practice
Dr. Amelia R. Cohn is a licensed Naturopathic Doctor (ND) in:
• Oregon
• Washington
Medical diagnosis and treatment may only be provided to individuals physically located in Oregon or Washington at the time services are rendered and only after formal establishment of care, completion of required intake documentation, and mutual consent.
Participation in programs, memberships, consultations, or educational offerings does not automatically establish a doctor–patient relationship.

Unless explicitly stated in writing:
• No diagnosis is made
• No disease is treated
• No medical care is provided

6. Functional Health Testing & Analysis
Functional or lifestyle-oriented testing tools may be used to:
• Identify patterns and trends

Subscriptions
• Subscriptions are billed in advance on a recurring basis
• You may cancel renewal through your Account or by contacting the Company
• Cancellation stops future billing; prior charges are non-refundable unless required by law

Our SMS program may include appointment reminders, notifications, account information, marketing messages, and support responses.

• Message and data rates may apply
• Carriers are not liable for delayed or undelivered messages
Compliance
You agree not to:
